Disease Care Management

All Generations Home Care is committed to improving quality of client care through our Disease Care Management Program. Disease Care Management is, as a strategy, used to enhance the quality of care received by clients suffering from one or more chronic conditions and cardiovascular disease. Clients, families and their caregivers are given the information and tools they need to better understand and adhere to recommended treatments for medications, lifestyle modifications, and self-management. This will improve health and reduce health care service use and costs associated with avoidable complications such as emergency room visits and hospitalizations.
